    CareLink Community Support Services - Full CareFull Care Residences help adults with mental illness learn the skills they need to live independently and become fully integrated into the community. Individuals work with a primary counselor to develop a personalized rehabilitation plan that identifies the unique interests, needs, and goals of each resident, and then maps out specific interventions and strategies. Family members play an important role in an individual’s care and are invited to resident treatment meetings, program parties, and open house events. Regular contact with residents’ support networks is strongly encouraged. Torrey House specializes in supporting adults with schizophrenia and similar serious mental illnesses. While each resident of Torrey House receives individualized care, Torrey House also promotes social activities and group therapies. Residents attend group sessions focused on psycho-education, health and nutrition, interpersonal relationship building, and arts and crafts. Residents participate in community events to promote socialization and integration with their local community. Case management services are provided to seniors who live alone with no family or caregiver support. This service is often packaged with Homemaker Services. Homemaker Services: When the care of the home becomes too much for the elderly or disabled individual, homemaker, meal, laundry, and errand support is available. Through this program Certified Home Care Aides assist with food shopping, meal preparation, laundry, general errands, and light housekeeping. This service is often packaged with Personal Care Assistance. Respite Services - Trusted Support and a Break: Respite Services offer Care for the Caregiver by providing a much needed break to enjoy family activities, pursue personal interests, relax for awhile or attend to family errands and personal responsibilities. Through this service, Certified Home Care Aides may provide personal care, homemaker services or companion services. Respite services affirm and encourage the primary caregivers and help them return to care giving with confidence, knowledge and a new energy. Companion Services - Avoid Isolation: For those who live alone and are homebound, the Companion Services of VNA offer important personal contact, socialization, and activity during the day with their certified home care aides. Case Management Services: The Case Management Services of VNA offer assistance in establishing a definitive Home Care Plan as well as coordination of necessary services. This service offers peace of mind for those who are responsible for the care of a loved one. Senior Health Specialist Program: Registered Nurses provide Health Specialist services to community based programs for seniors. Services include health check-ups, health screenings such as blood pressure screening and health counseling, specific disease management education, nutrition teaching, and medication review. The program includes valuable communication with the senior’s family. VNA caregivers are supervised and trained by registered nurses. They are employees of the VNA. All necessary employment taxes and insurances are covered by the agency. Each Home Care Aide is screened for drugs, alcohol, and background criminal activity.     Vocational Rehabilitation ServicesThe Pennsylvania Office of Vocational Rehabilitation, or OVR, provides vocational rehabilitation services to help persons with disabilities prepare for, obtain, or maintain employment. OVR provides services to eligible individuals with disabilities, both directly and through a network of approved vendors. Services are provided on an individualized basis. The OVR counselor, during face-to-face interviews, assists customers in selecting their choice of vocational goals, services and service providers. An Individualized Plan for Employment (IPE) is developed, outlining a vocational objective, services, providers and responsibilities. Certain services are subject to a Financial Needs Test (FNT) and may require financial participation by the customer. Counseling and guidance, diagnostic services, assessments, information and referral, job development and placement, and personal services such as readers or sign language interpreters are provided at no cost to the individual. Also, by law OVR customers receiving Social Security benefits for their disability (SSI, SSDI) are exempt from OVR’s Financial Needs Test. Statewide there are 21 District Offices staffed with trained, professional Vocational Rehabilitation Counselors which serve Pennsylvania in all 67 counties. The Hiram G. Andrews Center in Johnstown provides vocational training and comprehensive rehabilitation services to people from across the state. OVR’s Bureau of Blindness and Visual Services also provides specialized services to blind and visually impaired individuals. These services are designed to increase an individual’s independence and employability.
