Community Care manages the mental health and substance abuse services offered to Medical Assistance recipients through the HealthChoices Program. Services may include: Answering questions about the behavioral health plan and benefits and treatment programs. Help in finding treatment for mental health or drug and alcohol problems. Connects clients to transportation services for appointments to mental health or drug and alcohol appointments. Provides health coordination for all client's procedures.
