Brook Lane Partial Hospitalization Program for Adults and Children and AdolescenAdult Day Program: A week long program that provides individuals 18 years and older a structured therapeutic day program and they return home in the evenings. Compeer LancasterMatches volunteers in one-to-one relationships with adults who are recovering from mental illness. Their friendship provides structured opportunities for socialization which decreases loneliness and increases well being and confidence in the clients. Some opportunities include meeting for coffee, visiting local museums, going out to eat and taking walks together. Volunteers commit to four hour a month for one year. Other volunteer opportunities include making a 15 minute phone call each week to an individual waiting for one-to-one friend. Free training and ongoing support is provided. Options include: community homes, small group residences with round the clock, in-home assistance; semi-independent or supported living arrangements for individuals who need less than 24-hour per day in-home assistance and who live in their own homes or apartments; and family living, for those desiring a more family-oriented living situation. - Life-Long Supports: Horizon House also offers community integrated employment, financial planning and management services, and retirement planning. The Active Retirement Program provides specialized supports to people with mental retardation living in nursing homes. Human Services - Adolescent Partial Program ImPACTAdolescent Partial Hospital Services ImPACT provides an intensive psychiatric day treatment program for socially or emotionally impaired adolescents who are exeriencing difficulties in school and at home. The Program also provides an outpatient alternative to impatient hospitalization, or may serve as a transition program following impatient hospitalization. Treatment services include individual and group therapy, medication and monitoring and family consultation. An educational component is provided in cooperation with the Chester County Intermediate Unit. Lenape Valley Foundation - Transitional Outpatient ServicesTransitional Outpatient Services offers a multi-modal, multi-disciplinary structure for a variety of programs. The program has a variable length of treatment and offers flexibility in scheduling as symptoms are alleviated and the individual is able to resume their typical activities. Transitional Outpatient Services is more intensive than traditional outpatient and is designed to achieve stabilization and resolution of psychiatric and chemical dependency conditions. This level of care is specifically available for those adult clients who do not require the intense level of psychiatric care provided by inpatient or partial hospitalization programs. Mentally Ill Offenders Initiatives - Lycoming CountyLycoming County Adult Probation collaborates with the Lycoming/Clinton Mental Health/Intellectual Disabilities Joinder (MH/ID) to provide services for mentally ill offenders. The Prison/Mental Health treatment team includes a MH/ID contracted psychiatrist, a Mental Health Caseworker, and an MH/ID psychologist. The contracted psychiatrist schedules weekly visits to assist in medication checks and address the needs of the seriously mentally ill. The Mental Health Caseworker meets weekly with mentally ill inmates who experience depression, anxiety, and adjustment problems. The caseworker also helps identify inmates who may have had previous mental health issues. The MH/ID psychologist assists in a suicidal inmate observation review process.
