|Family Care for Children and Youth - Leesport - Family Reunification||Family Reunification/Family-Based Services is the planned process of reconnecting children in out-of-home foster care with their biological/legal families. The purpose of our Family Reunification/Family-Based Services program is to help young people achieve and maintain, at any given time, their optimal level of reconnection, from full reentry into a family system to other forms of contact which affirm family membership. To help accelerate the reconnection process, frequent case manager contact is arranged and in-home services are provided by a reunification coordinator. Improved family relationships, communication, and family functioning are key elements of Family Reunification/Family-Based Services.
Family Reunification/Family-Based Services combines the strengths of Family-Based Services with in-home foster care services. |Foster Care and Adoption Services, Children's Home of York||Children's Home of York's Permanency Program provides a variety of services for children and their biological, foster, and/or adoptive families from birth through the age of 18. Our structured, therapeutic family environment offers intensive support with the goal of helping children in our care secure a permanent family as quickly as possible. We work with a diverse pool of resource parents to provide kinship care and a variety of Permanency support services throughout Pennsylvania. The Children's Home of York maintains an on-going process of recruitment, screening, and training which enables us to offer diverse geographical and racial/ethnic environments essential to the successful placement of children and sibling groups of all ages who have a variety of emotional and psychological needs. In addition, we have two offices to provide regional support to our Resource Parent pool.

|Intensive Family Reunification Services-Family Service Association of Northeastern PA||The primary goal of the program is to implement brief, intensive, family-focused reunification service for families who have children placed in foster or kinship care in Luzerne County. The program is designed to reduce the amount of time children spend in out-of-home placement by strengthening families, building resources, addressing parenting concerns, and creating a plan for reunification. The team builds a child-focused partnership between the family, the community and human service agencies in order to support the family and facilitate planning for the safe reunification of children with their birth parents. |Lycoming Children and Youth Services - Foster Care / Adoption / Shelter Care Services||Works in cooperation with the community, families, and other organizations to guarantee health, safety, welfare, and social growth to children and youth. Case managers work in cooperation with schools, police, medical personnel, and other human service organizations to keep families together. |Progressive Life Center - NIA Foster Care||The NIA foster care program provides comprehensive and intensive child placement services, which include clinical and case management activities. This program serves children from birth to twenty-years old. The ultimate goal of the NIA program is to promote and achieve permanency (reunification with parents and/or family members, adoption or transition to independent living).
