Description of Service
A program working with people experiencing homelessness having the express goal of moving to permanent housing. All programs are housed at 105 E King St. Within the program, they serve many populations, including but not restricted to Veterans through our Veterans’ Victory House, families and households through the Emergency Shelter program.
Each household is assigned a case manager who works with each individual or family to develop an Individualized Housing Plan, again with the ultimate goal of achieving permanent housing. The plan, worked on together with the family or individual includes, among other items, a budget and a potential referral to a rapid re-housing program. Emphasis may also be placed on education, employment and/or recovery needs.
Referrals, depending on program, come from 211/CHART, the Lebanon VAMC and various other sources
A one on one assessment by a staff member at TLC
Homeless individuals, couples and families experiencing homelessness; Need to be independent and be able to perform one's activities of daily living; Must be physically and mentally independent; Must have a strong desire to get permanent housing
