Description of Service
Provides care and support for individuals of any age and their families coping with serious illness, end of life and loss. Services are provided by a team of professionals, including doctors, nurses, social workers, chaplains, bereavement counselors, complementary therapists and volunteers, with a focus on improving quality of life and patient dignity, while also provide family support. Hospice and palliative care is available for adults, teens and children with any serious illness:
- Clinical staff on-call around the clock and 24-hour admissions
- Physical, emotional and spiritual support to patients, caregivers and family
- Care is provided at home, in a nursing home or senior care facility, in a hospital and at Hospice & Community Care's two Hospice Inpatient Centers.
Bereavement support is available through the Pathways Center for Grief & Loss for adults, teens and children, regardless of whether hospice care was provided for a loved one.
