Description of Service
We are a home visiting program whose mission is to prevent infant mortality. To that end, we provide case management and information and referrals to families for community-based health and social services. We emphasize prenatal care, support for low birthweight infants, breastfeeding, health education, family planning, and other support services that assist in the reduction of infant mortality. We also provide postpartum depression screening and referral as well as breastfeeding, childbirth prep, and parenting classes for mothers. We also offer parenting classes for eligible fathers as well as case management services for them. We also offer some baby safety equipment for eligible participants.

Mothers should be pregnant or have children under 24 months of age. The male partners of the mothers we serve are also eligible for services. There are no income guidelines.
We have access to translation services for any needed languages.
