Description of Service
Provides a range of housing options for those with serious and persistent mental illness, including both short-term, long-term and independent housing.
-Community Residential Reabilitation (CRR)
Recovery based setting that is transitional and rehabilitative in purpose. Residential staff members assist residents in building skills that are necessary to move to a more independent setting. Residences consist of a cluster of shared apartments located throughout the community and support is provided at a Maximum level. Staff members are available on an on-call basis during the overnight hours. (Serves 14 individuals.)
-Long Term Housing Programs
Residential staff members assist residents with recovery goals, providing support in a quiet country setting. Residents of this program are able to live in the program for as long as they wish, given residential guidelines. (Serves 5 individuals.)
- Assisted Permanent Housing Program
Residents living in this program have achieved most of the skills necessary to live independently and continue to work on their recovery goals. Residential staff members provide support a few hours a week and as requested. There is no maximum length of stay. (Serves 8 individuals.)
-Independent Lease Program
Residents living in this program have achieved most of the skills necessary to live independently and continue to work on independent recovery goals. Residential staff members provide support as requested and aid in making connections to community services. There is no maximum length of stay. Tenancy is maintained by adherence to lease conditions. (Serves 14 individuals.)
