Description of Service
Standardized and coordinated assessment and referral service for everyone who is seeking services from the network of homeless services.
Services include:
- Assist a household that cannot otherwise be diverted or prevented from homelessness connect with the shelter that best fits the household's needs and goals.
- Assist persons experiencing homelessness to be quickly re-housed and stabilized.

Walk-in during business hours or call 2-1-1 East for CHART information 24/7 and pre-screening and referral Monday-Friday, 7:00am to 5:00pm
