Description of Service
Companion services are provided to individuals living in private residences for the limited purposes of providing supervision and minimal assistance that is focused solely on the health and safety of the adult individual (18 and older) with mental retardation. This service is not available to people who are residing in Unlicensed or Licensed Residential Habilitation settings. Companion services are
used in lieu of habilitation services to protect the health and welfare of the individual when a habilitative outcome is not appropriate or feasible.
Ask for LiNKS to Everyday Life Program. PA's Consolidatedor P/FDS Waiver. Medicaid number, birthdate, social security number and other demographic and functional information will be taken at the initial assessment. Potential participants will need to authorize their Supports Coordinator to share information with our program. Before services can be authorized, an ISP meeting will need to be held.
People of any age with Intellectual / Developmental Disabilties on PA's Consolidated or P/FDS Waivers.
