General Information
Description of ServiceProvides care coordination for individuals being prescribed the medication Buprenorphine and Vivtrol for opiate dependence. Recovery Coordinators provide assessment, referral, recovery planning, care monitoring, medication monitoring, and interface with prescribing physicians and outpatient treatment programs to provide a team approach.

Client Information
EligibilityAdults between the ages of 18-65 who are engaged in opiate addiction or those already on Buprenorphine. They work with individuals on medical assistance, self-pay, or those eligible for Lebanon County funding.

Types of FeesFees vary-No fees is on medical assistance or county funded. Self-pay clients will get fee schedule during phone intake.
