Description of Service
VA Pittsburgh Healthcare System,s Dementia Caregiver Support Program is and innovative model of a patient-centered alternative to institutional extended care.
This program utilizes a multidisciplinary team approach involving a registered nurse, social worker, and psychologist. Team members utilize Telehealth technology, phone interventions, and a caregiver telephone support groups monitor and provide interventions or caregivers. The primary goal of this program is to reduce caregiver burden and stress, provide caregiver education and resource identification, and improve overall quality of life for both caregivers and veterans.
ATo have a Veteran and caregiver evaluated, a provider can place a consult in CPRS to initiate a review
for program enrollment or call 412-328-5991.

The Dementia Caregiver Support Program can serve caregivers within a 60-mile radius of VAPHS.

Eligibility
The veteran must be enrolled in VAPHS and be seen at least annually. The veteran must have a dementia diagnosis. The primary caregiver must be living in the same residence and not be a paid caregiver.
