Description of Service
Provides a variety of outpatient psychiatric and crisis intervention services for children, adolescents and adults. Child Mental Health Services including Child Blended Case Management; School Based Partial Hospitalization Services; Family Based Behavioral Health Services; Student Assistance Program; Transitional Aftercare; Family Reintegration Services, and CASSP. Early Intervention Service Coordination for ages 0-3 years of age and Supports Coordination for those with an intellectual disability. Adult Mental Health Services include Adult Blended Case Management; Psychiatric Rehabilitation Services; Certified Peer Specialists; Residential, and Vocational Services.
Walk in intakes
Proof of identification- photo ID, birth certificate; insurance card(s); Social Security Number; Date of Birth.
For Children: Legal guardian and child under the age of 14 must be present. Children 14 or older must be present and may choose to have legal guardian present.
HSC provides information and referral for mental health and developmental services.
