Description of Service
Help homeless or at-risk veteran families quickly regaining stability in permanent housing after experiencing a housing crisis and/or homelessness.
- Case management (assessment, housing stabilization plan/referrals)
- Assistance in obtaining VA benefits and other public benefits/community resources and services as available
- Temporary financial assistance may be provided as last resort based on assessed individual need

- Must be a veteran or veteran family (HOH or partner of HOH must be a veteran; excluding those who have a dishonorable discharge and those who have served in the National Guard unless called to Active Duty in time of war as evidenced by a DD 214 - Very low-income: Gross household income must not exceed 50% of the median income guidelines - Must be homeless or at imminent risk of becoming homeless (must have a formal eviction notice)
