You must bring photo ID, also you must provide proof of residence that you live in the service area, for example a water/gas/electric/etc. bill with your name and address on it. Access/EBT card, or proof that you are receive public assistance. Two forms of identification for each child age 0-12 that you register, e.g. birth certificates, medical cards and social security card (one of these must show their date of birth)
At public distribution, only children's legal guardians can obtain toys -- legal guardians cannot send others in their place. Do not call Uniontown Mall for sign-up information.
