Description of Service
Provides support to help older adults be able to continue living at home. Based on an individual’s need, LIFE can provide: comprehensive medical management by an interdisciplinary team of healthcare professionals; an on-call nurse and physician available 24 hours a day; adult day services; personal care; home care; housekeeping and laundry; rehabilitation therapies; transportation to the LIFE center and medical appointments; medications and medication management; meals; medical equipment; and dentistry, optometry, and audiology.
Call to obtain information on the LIFE Program and to set up an appointment or tour. Walk ins are also welcome.
Intake staff will provide information on the documents needed to begin the application process. Examples of documents that may be needed are: Medicare card, other insurance cards, documents to verify financial eligibility, photo ID, power of attorney documents, etc.
A person is eligible for LIFE Lawrence County if he or she is:1. 55 years of age or older2. Having difficulty managing his or her own health-related needs independently at home and meets a level of care determined by the Lawrence County Office on Aging3. Able to live safely in his/her home or the home of a caregiver with LIFE services.

Hours of Operation
Adult Day Center is open weekdays, 8:30am to 5:00pm, but care is managed 24/7 via on call nursing. Home care services are available seven days per week.
