General Information
Description of ServiceThe Obra Waiver Program, is a Home and Community Based Waiver program that helps those with developmental physical disabilities to live in the community and remain as independent as possible. Available services may include:
  • Adult day services
  • Assistive technology/specialized medical equipment and supplies
  • Community Integration, which helps consumers with socialization and adjustment to surroundings
  • Community transition services
  • Personal Assistance Services such as bathing, transferring cooking, cleaning, and shopping
  • Education services
  • Environmental Accessibility Adaptations, a waiver that pays for home modifications not covered by insurance
  • Extended state plan - visiting nurse, physical therapy, speech therapy, occupational therapy, and behavior therapy
  • Non-medical transportation, i.e. bus tokens
  • Personal Emergency Response Systems (PERS units)
  • Prevocational services
  • Respite services
  • Service Coordination by a coordinator who serves as the consumer's "go to" person
  • Supportive employment services

EligibilityGeneral eligibility requirements:
  • Be between the ages of 18 and 59. Individuals who are currently in the waiver prior to 7/1/2006 will age in place. Individuals age 60 and older will be referred to the Aging Waiver.
  • Have a severe developmental physical disability requiring an Intermediate Care Facility/Other Related Conditions (ICF/ORC) level of care.
  • Consumer's disability results in substantial functional limitations in three or more of the following major life activities: mobility, communication, self-care, self-direction, capacity for independent living, learning.
  • Meet the financial requirements as determined by the local County Assistance Office.
Other related conditions (ORCs) include physical, sensory, or neurological disabilities which manifested before age 22 and are likely to continue indefinitely.
