General Information
Description of ServiceThe Independence Waiver Program, under the Community Services Program for Persons with Physical Disabilities (CSPPPD), gives assistance to adults with severe physical disabilities to enable them to live or remain in the community with as much independence as possible. Services available may include:
  • Adult daily living services
  • Assistive technology/specialized medical equipment and supplies
  • Community integration services, which helps consumers with socialization and adjustment to surroundings
  • Community transition services
  • Education services
  • Environmental Accessibility Adaptations, a waiver that pays for home modifications not covered by insurance
  • Extended state plan - visiting nurse, physical therapy, speech therapy, occupational therapy, and behavior therapy
  • Non-medical transportation, i.e. bus tokens
  • Personal Assistance Services, such as bathing, transferring cooking, cleaning, and shopping
  • Personal Emergency Response Systems (PERS units)
  • Respite Care
  • Respite Care Services
  • Service Coordination, with a coordinator who serves as the consumer's

EligibilityGeneral eligibility requirements:
  • Be a Pennsylvania resident
  • Meet the level of care needs for a Skilled Nursing Facility or a Special Rehabilitation Facility for the Medicaid Waiver. Those who do not meet the Skilled Nursing Facility or Special Rehabilitation Facility level of care MAY qualify for the PA State funded Act 150 Program
  • Be between the ages of 18 - 59
  • Be capable of hiring, firing, and supervising attendant care worker(S); managing your own financial affairs; and managing your own legal affairs
  • For the Medicaid Home and Community Based Waiver Services Attendant Care Program, meet the financial requirements as determined by your local County Assistance Office.
  • Have a medically determinable physical impairment that is expected to last of a continuous period of not less than twelve (12) calendar months or that may result in death
To take advantage of the Attendant Care Act 150 Program, you may be assessed a minimal co-payment. This co-payment is based on your income and will not be more than the total costs of services.
