Description of Service
The Attendant Care Program (AC) Waiver enables adults with physical disabilities who are determined to need nursing facility services, but who choose to remain in their own homes, to continue to live in their homes and communities with support and services. Services offered include:
Financial Management Services (FMS) who manage the payroll for attendants, workers' compensation, taxes for attendants, and most items dealing with the financial issues involved with this program
Personal Assistance Services such as bathing, transferring cooking, cleaning, and shopping
Personal Emergency Response Systems (PERS units)
Supports Coordination Services by a coordinator who serves as the consumer's "go to" person.
Consumers who do not qualify for the Medical Assistance Waiver either because they are not nursing facility eligible or their income is over the financial criteria set by the County Assistance Ofice may receive services under ACT 150 with a fee based on household income and the number of people living in the household.

Roads to Freedom does not assess eligibility for consumers to enter into the program. The local County Assistance Office make assessments, and consumers who do not qualify for the Medical Assistance Waiver either because they are not nursing facility eligible or their income is over the financial criteria set by the County Assistance Office may receive services under ACT 150 with a fee based on household income and the number of people living in the household.Eligibility Requirements:
Be a resident of Pennsylvania
Meet the level of care needs for a Skilled Nursing Facility or a Special Rehabilitation Facility for the Medicaid Waiver
Be 18 to 59 years of age
Be capable of hiring, firing, and supervising attendant care worker(s)
Be capable of managing your own financial and legal affairs
For the Medicaid Home and Community Based Waiver Services Attendant Care Program, meet the financial requirements as determined by your local County Assistance Office.
Have a medically determinable physical impairment that is expected to last of a continuous period of not less than twelve (12) calendar months or that may result in death
To take advantage of the Attendant Care Act 150 Program, you may be assessed a minimal co-payment. This co-payment is based on your income and will not be more than the total costs of services.
