Description of Program
Home health care is provided by nurses, therapists and home health aides to help patients recover from illness or injury. Serves individuals from infancy to adulthood.
Hospice care can be provided at home, in long term care facilities or in St Luke's Hospice House in Bethlehem. Hospice also provides bereavement support and guidance for anyone dealing with the loss of a loved one, regardless of where their loved one died. Bereavement support services are free-of-charge, open to all and include support groups, workshops for adults and children, lectures/discussions and services of remembrance. Ryan's Tree is a six-week guided peer support program designed for children and adolescents, age 5 - 18, who are grieving the loss of a family member or friend. Nurse-Family Partnership (NFP) provides visiting nurses who work with low income, first time mothers on parenting, nutrition and birth control from the beginning of their pregnancy until their babies are two years old.
PATH program provides visiting nurses to families who need support with daily tasks and parenting.
